First Visit
Your initial appointment will consist of any necessary X-rays, diagnosis and a consultation regarding your treatment options. If your treatment plan calls for root canal treatment, we will schedule another appointment for treatment. If you are in extreme discomfort, we will do our best to see you the same day that you call on an emergency basis. Appointments will then be schedule to complete treatment.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays if applicable
- A list of medications you are presently taking
If you have dental insurance, bring your current insurance card. If you don’t have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.
IMPORTANT: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit. Parents are not allowed in the treatment room unless requested by the staff. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
X-Rays
If your dentist or physician has taken X-rays, please have them forwarded to our office as they may be helpful in our diagnosis. A digital radiograph can be emailed to us. If additional films are necessary, they can be taken at our facility.
